Abstract
The article is devoted to optimisation of the treatment and rehabilitation protocol for patients with severe generalized periodontitis by reducing the number of surgical procedures and shortening the total duration of comprehensive treatment. The aim of the study was to substantiate the possibility of simultaneous implantation after multiple tooth extraction in this category of patients. The study included 17 patients whose surgical treatment was carried out in one or two procedures. During treatment, 145 teeth were removed, 33 closed sinus lifts were performed, and 164 Straumann implants were placed. Osteoplasty was carried out using collagen materials without membranes, and subsequent prosthetic rehabilitation, with the exception of one case, was performed with fixed ceramic constructions. It was established that on average 8 teeth were removed and 8 implants were placed during one operation, while the total number of surgical manipulations averaged 17 per intervention. Osseointegration was achieved in 99% of cases, and in 70% of patients prosthetic treatment was completed already 3–4 months after surgery. No significant differences were found between the effectiveness of delayed implantation and simultaneous implantation. It was concluded that the proposed algorithm makes it possible to provide comprehensive treatment for patients with severe generalized periodontitis within a shortened time frame, ensuring high effectiveness, a predictable clinical result, and the possibility of early prosthetic rehabilitation.
