ORGANIZATION OF A NEW MODEL OF SURGICAL CARE FOR NEWBORNS WITH CONGENITAL MALFORMATIONS IN THE KARAGANDA REGION IN THE PERIOD FROM 2015 TO 2023

Abstract

The article is devoted to improving surgical care for newborns with congenital malformations in the Karaganda region. The paper considers organisational changes aimed at reducing infant mortality by excluding the transportation of newborns from one hospital to another and introducing treatment in perinatal centres with the participation of neonatal surgeons and neonatologists. It is noted that until 2017, medical care for this group of patients was provided only in the Regional Children’s Clinical Hospital, but transportation was associated with the risk of disruption of the “heat chain”, deterioration of the newborn’s condition, progression of metabolic disorders, and postoperative complications. The article presents the results of a retrospective analysis of children with congenital malformations operated on in 2015–2023 and shows that the introduction of a new model of care made it possible to significantly reduce mortality. During the reporting period 2018–2023, the mortality of newborns with congenital malformations operated on in maternity hospitals of the Karaganda region decreased from 53.8% to 13%. The importance of continuity of the treatment process, reduction of the time before surgery, and development of neonatal surgery in perinatal centres as the most important conditions for improving the survival of newborns with this pathology is emphasised.
