Application of a patented urine collection method for testing antibiotic resistance in pregnant women with urolithiasis: A comparative cohort study

Abstract

Urolithiasis during pregnancy is one of the most significant urological disorders, associated with severe obstructive pyelonephritis, a risk of preterm birth and spontaneous abortion. The accuracy of microbiological diagnosis in these conditions is limited by the level of contamination of the biological material using standard urine collection methods – resulting in up to 30% false-positive results – which highlights the need to develop an aseptic technique. The aim of the study was to determine and compare the microbiological characteristics and antibiotic resistance profiles in two groups of pregnant women with urolithiasis as part of a retrospective cohort analysis. A retrospective cohort study (2020-2025) was conducted involving 271 pregnant patients aged 17-38 years (mean age 25.5 years) with overt obstructive pyelonephritis associated with urolithiasis. Two methods of upper urinary tract drainage were examined: in group 1 (n = 105), a patented double-lumen stent-catheter with aseptic urine collection directly from the obstructed kidney was used; in group 2 (n = 166), standard JJ stenting with urine collection was employed. It was found that the leading uropathogen in each group was Escherichia coli – 40.0% and 45.2%, respectively. Superiority in the susceptibility profile was confirmed in Group 1: E. coli susceptibility to ceftriaxone was 100% versus 76.2% (Δ 23.8%), and to ampicillin – 94.7% versus 71.4% (Δ 23.3%). The greatest inter-group difference was observed for Enterococcus spp. with regard to ciprofloxacin: 86.4% versus 55.6% (Δ 30.8%). It was established that fosfomycin (100%) and amikacin (90%) exhibit the highest levels of overall activity, confirming their priority status in obstetric practice. It has been confirmed that the patented dual-stent catheter simultaneously provides decompression of the upper urinary tract and the collection of uncontaminated biomaterial, thereby optimising aetiological therapy in pregnant women with urolithiasis in Kyrgyzstan
